Haliburton Highlands Health Services is taking part in a provincial pilot project to improve care through innovation.
The province says it is providing $2.5-million over the next three years to five hubs, to enhance service integration so they can meet the unique needs of rural communities.
The goal is to connect modern services and coordinate transitions between caregivers and doctors, and make it easier for patients and their families to understand and access their health care choices.
The aim is for these health hubs to become fully integrated healthcare delivery systems by 2018.
