Haliburton Highlands Health Services wants to find out how patients are being cares for after leaving the hospital.
A provincial advisory body for healthcare is conducting the survey.
H.H.H.S. President and C.E.O. Carolyn Plummer says the survey is for anyone who has spent at least a night in the hospital in the last three years.
Plummer adds the information obtained from the survey will help health services shape its future programs.

The survey asks what types of support patients need when they return home and some of the challenges they face after an extended hospital stay just to name a few.
