Haliburton Highlands Health Services is now part of a one of a kind program. The GAIN (Geriatric Assessment and Intervention Network) Team has officially launched and it’s one of only 12 in the province. The initiative was started by the Central East Local Health Integration Network.
The service is available to frail seniors and is meant to help them live in their homes longer. The team is made up of people from different services including geriatrics to make sure the clients get a balanced care plan.
They’ll also be doing home visits so they can look at how a senior’s home is affecting their health.
The team started taking clients in July, and they already have 30. HHHS CEO Varouj Eskedjian says their goal is to have 60 by the end of this year.
